By submitting this intake form on behalf of your union/association, you confirm and agree that:

Authority to represent the union. You are an authorized representative of the union/association and are authorized to submit information and communicate regarding the applicable collective bargaining agreement (“CBA”) and related union materials.

Permission to use union name and logo. You have authority to allow MyUnionBoss to display and use the union/association’s name and logo solely for identification/branding purposes in connection with the contract listing/registry, the union’s contract page, and the associated MyUnionBoss experience.

No liability or guarantees. The union/association does not assume liability for the accuracy, completeness, interpretation, or outcomes of content displayed or produced by MyUnionBoss. Allowing use of the union/association logo does not constitute endorsement, certification, or any guarantee of accuracy.
